How to use the Client Information Form
- Enter a name, email address and your choice or request. A phone number is optional.
- Choose Envoyer to save the submission locally in this browser.
- Return to this page on the same browser and device to view saved submissions or download the complete Excel file.
Local storage and privacy
Submission records are saved in IndexedDB in your browser on this device. They are not uploaded to a server. Clearing browser site data or changing devices removes access to these local records.
